FOSTER-TO-ADOPT Agreement Form



The goal of this agreement is to allow loving families to welcome rescue pets into their home
sooner rather than later and to ensure these pets will be spayed or neutered as soon as they
are old enough or healthy enough. Pet over-population is a tragic occurrence in our society and
we want to do all we can to prevent euthanasia and suffering we see in shelters across the
country.

The Animals In Need Fund Foster-to-Adopt Program was created to place unaltered cats or
dogs into approved forever homes in a foster scenario prior to official adoption. The Foster-to-Adopt
Program allows approved applicants to welcome an unaltered pet into their home until the animal is
old enough or healthy enough to be spay/neutered by a licensed veterinarian with the Nipomo
Dog and Cat Hospital. If the Foster-to-Adopt family is accepted, they will foster the cat or dog until
they have been spayed/neutered, then the adoption contract can be finalized.


In order to be accepted into this program, Foster-to-Adopt applicants must have an approved
foster application. In addition, applicants must sign this Foster to Adopt Agreement, which
requires compliance with all of the Foster-to-Adopt terms and conditions, including the adoption
procedures.

I understand that:

As a Temporary Foster Home for AINF, with the intention of adopting the fostered cat or dog, I

agree to comply with the following conditions and requirements:


1. The pet is and shall remain under the care of AINF until it is spayed or neutered and the

adoption is finalized. If for any reason the adoption is not finalized, I understand that

upon the request of the AINF, the pet shall be relinquished to AINF and the adoption fee

may be refunded.


2. I will provide the pet with good care, including but not limited to, proper food, fresh water,

shelter, exercise, grooming, training and medication when required.


3. I will comply with all instructions received from the AINF and will not deviate from any

instructions as to the care and maintenance of the pet without consulting with a

representative of the AINF.


4. I agree to maintain the animal as my own household pet and companion.


5. I agree not to abandon, trade, give away, or sell the pet. If I no longer desire or am

unable to keep the pet within the terms of this agreement, the pet must be returned to

AINF. The pet is NEVER to be turned over to an animal shelter or passed to another

owner.


7. I agree to keep the pet under my control and not let the pet roam freely outside of the

home. Cats or kittens must be kept indoors.


8. I will provide a safe and loving home environment.


9. I agree to the following in regard to veterinary care:


a. I understand that AINF is providing appropriate veterinary care to date and is in

good health except for the following medical conditions about which I have been

informed: (applies only to pets with special needs)      

b. If the pet needs medical attention, a member of AINF must be contacted

immediately and must approve all vet visits. Said visit must be with one of AINF’s

approved vets with whom we work. If FOSTER takes foster pet to an unapproved

veterinary hospital for any reason, FOSTER agrees to be responsible for all

medical bills incurred as a result of said unapproved vet visit, except in cases of

emergency when the pet’s life may be in danger.

c. AINF will make arrangements for spay/neuter surgery and I agree that I will take

this pet to its spay/neuter appointment and for any other necessary veterinary care

as directed by AINF unless other arrangements have been made by mutual

agreement. There are no exceptions to the requirement that the animal must be

spayed/neutered before adoption is complete.


10. When the pet has been spayed/neutered, the adoption contract can be approved and

any necessary veterinary records/adoption folder will be provided to me. I agree that I

am responsible for the cost of any veterinary care after this adoption is complete.


11. This agreement is not transferable. If I am unable to care for the pet, I will immediately

return the animal to the AINF.


12. I give permission to AINF to enter upon my premises at anytime upon reasonable

notice for the purpose of determining whether or not I am complying with this

Agreement, and to remove the pet, in the event that I violate the terms of this

Agreement.


13. I agree not to attempt to hold the AINF responsible for any damages which the pet may

do to any person or property. I specifically release AINF of any responsibility pertaining

to any damage to property or personal injury or any occurrence relating to the pet and

shall hold AINF harmless regarding any damage or injury(ies) of any nature

whatsoever, and agree to notify AINF immediately of any damages and/or injury(ies).


14. I recognize this is an Agreement and have signed (e-signed) this Agreement freely and

voluntarily.

Pre-surgery instructions for your foster pet: No breakfast the morning of the

scheduled spay or neuter appointment. You can offer dinner the night before, but

please remove access to all food before heading to bed. Access to water is ok.

Please bring your foster cat or dog to the Nipomo Dog & Cat Hospital the

morning of your scheduled appointment (we will arrange for a day that works for

your schedule) for drop off between 8am – 8:30am (unless other timing has been

arranged). You will be able to pick up your pet from Nipomo Dog & Cat Hospital

later that same day. Typical pick-up time is between 3-5pm.
